Important Membership Information
- Membership Dues: Typically assessed on a per-license, annual basis. Please confirm the most current dues schedule on the Member Center.
- Eligibility: NJ-based, SAMHSA-certified Opioid Treatment Programs (OTPs). Each licensed program location should complete its own membership form.
- How to Use These Forms: Download the PDF to print and complete by hand, or use the Word version for electronic completion. Return the completed form with payment or according to the instructions provided by NJATOD.
If your organization prefers to process membership online, we recommend using the secure registration and payment options available through the NJATOD Member Center . These downloadable forms are maintained here as part of NJATOD’s resource repository for agencies that require internal routing, signatures, or hard-copy documentation.
